I have installed DNN 3.2.2 with windows authentication. i have installed form master module (FormMaster_DNN30_01.05.02_PA.zip) in my dnn. My requirement is that we have a form having field called name , emailid and comment field. I have disabled the name and email id field so that user can not fill these fields but when they open the form name field should take from the windows authentication and email should take from the database and user should ONLY put their comments.

Please let me know is it possible to fill the name text filled with windows authentication value and emailid from database .
