Version FORM MASTER 1.5 for DNN 3

Hi,

I did the installation and everything went smoothly, when I add the module to the ContentPane I get this error

A critical error has occurred.
MinMax persistance type of cookie requires a ModuleId

Also from the action menu I only get , Import Content, Export Content, Help, Settings, Delete, Move. No way to build a form at all

Nothing at all like the tutorial you have on your website.

Any idea why or how I could fix this?

this is a well tested module with 100's sold. Please post the full exception from the log viewer.
