General formatting issues:

1.  I have a form located here:  and I want to completely format it:

   a.  I attempted to make it 2 columns but the second column ended up only being around 25% the width of the 1st column when displayed... i didn't see an area where I could adjust columns individually

   b.  I was to add padding ONLY in between primary questions.... how do I do this?

   c.  I want to add different background colors per question... how do I do this?
