DNN Version:
Form Master Version:

Is tihis a new install?
Were there any error messages during Install?
Are you using an ObjectQualifier in the Web.config file?
Are you using something other than "dbo" as DatabaseOwner in the Web.config File?
Is the Legacy connection string in the Web.config file configured?
