After setting up the form the way you want it:
1. On the module action menu select Export
2. Copy the file that is created in /Portals/[PORTALID]/content.FormMaster. ... .xml to the target PortalID
3. To to Admin-->File Manager scroll to the bottom and click on synchronize files
4. Put a new Form Master Module on a page
5. On the module action menu select Import
6. Select the file you copied in #2
