I have been working hard trying to implement a sample form for use in the latest version of DNN 4.  I just purchased your product with src.  However, I have been unable to determine why this error ocurrs. 

Using the included sample form, Not all fields are saved to the database or available to the tokens in the custom template.  Please review.

Consistently, these fields do not post:   MAILDROP, FILEB, FILEC, VENUEC, CONTACTNAME

You found a UI and logic bug. Uncheck Exclude from Email & Data on

 

Spacer1, Spacer2, Spacer3, and the Submit Button and you will be all set.

 

Labels and buttons are excluded automatically and the checkbox should not be available for those types.

 

I will include the fix in the next patch.
