Survey spanning multiple pages

DNN 5.0.0.0

I need to make a form/survey that spans multiple "pages". I have 60 questions (6 sections of 10 questions each). I don't want all 60 questions on the same page, but i want them all in the same form.

At the bottom of each section i want to put in a NEXT button, which takes the user to the next section. On the final section the NEXT button will be replaced by a SUBMITT button.

Is this possible?
If so how do i go about doing it?
Do i need 6 separate pages in my website and then somehow link each survey section together?
Please provide some instructions.

We will have a Sample Form set to show how to link multiple forms on the Form within a few days.

Hi MA,

I have had some issues with my DNN installation and have had to start again. I was hosting DNN5 on, IIS7, SQL2008 and SBS2008.  Being so new I think these platforms are a bit buggy are are not working together well. SBS2008 is especially difficult and complicating things. So I am reinstalling Server 2003 and going back to IIS6, SQL Express and DNN 4.9.

As a result of these problems I've not had a chance to begin working out the form solution.

I run a small company and have been working 14 hours a day 7 days a week for the past 3 weeks trying to get everything sorted. I am going to be out of the office for the next 10 days returning 9 Feb. Check back about 14 Feb. I should have it sorted by then and will post my solution.

This has not been addressed. The only offering is as posted in the link above and I have put together a simple example in this post.

A major release with this functionality will probably be not until the end of the year.

Does anybody know if another solution for this has been developed? I am looking for a way to have a form (about 100 questions) that is multiple pages.

I am a really big fan of the Form Builder module and ideally would not like to have to learn another module to accomplish this functionality.    Does anybody know if the developers are still considering multi-page functionality?

Development of Form Master 2011 will start in earnest in November 2010. Multi Page functionality will be included. A feature list will be posted within two weeks. I expect a release by the end of Q1 2011.
