Error on adding formaster 2008 module, DNN4

I get an error each time I add a Form Master 2008 module. "Cannot insert the value NULL into column 'SystemList', table 'bcw0818704172281.dbo.Lists'; column does not allow nulls. INSERT fails. The statement has been terminated." The module is there, and I can edit and enter data. When I try to "create table from form" I get another error "A network-related or instance-specific error occurred while establishing a connection to SQL Server. The server was not found or was not accessible. Verify that the instance name is correct and that SQL Server is configured to allow remote connections. (provider: Named Pipes Provider, error: 40 - Could not open a connection to SQL Server" I don't know that they are related. Help please.

Can you explain how your SQL Server is setup for DNN?
What version of DNN are you using?

Also can you please provide the exact verison of Form Master 2008 2.0.?
