Form Layout Problem - In Need of a Work-A-Round?

As i try to get more familiar with the inner workings of Form Master.
I've stumbled accross a layout problem of which i'm needing a work-a-round or suggestion.
Perhaps i'm missing something here.

I'm trying to layout a new form in Form Master that looks like the following:
And would like to know if it is even possible to layout the text boxes similar to
what i have listed via html below:

Form Master for DNN2 does not have very good layout capabilities however the only thing I can see which cannot be done here is the Addres and Contact Info heading, and the cell aligment for the textboxes will always be left.

You can always put the Address and Contact info first and put the line in the Form heading. Use the table class to format the talble cell as shown for the DNN3 online documentation and in numerous posts in the DNN3 version forum.
