Description:
When trying to view form results only FormDataID and FormResults Id show. But when only one record exists, the correct data shows.

Reslolution:
The problem above is specifiic to DNN sites running on any version of SQL Server 2005. Sorting for the "FOR XML" clause had changed between SQL Server 2000 and SQL Server 2005. Until the next release of Form Master 1.5, the problem can be resolved by running the Attached SQL Scrpt in Host-->SQL with the "Run as Script" checkbox checked.

Now that Form Master 1.5.1 is out, upgrading to the latest version will reslove the issue as well.
